Kite Runner Essay Human beings are morally ambiguous people. We are neither purely evil nor purely good, but often a mix. And maybe that’s why many of us are attracted to literature works with morally ambiguous characters such as The Kite Runner by Khaled Hosseini. The Kite Runner was set in Kabul, Afghanistan, proceeds to United States during the Soviet Union invasion, and then the setting goes back to Kabul when the Taliban rises in power. In this novel, Amir, to whom the whole story of the book is centered around, is a morally ambiguous character. Amir is a Pashtun boy; he betrays his friendship with Hassan, a Hazara son of Amir’s father’s servant. Guilt haunts Amir for years even after he had left Kabul and moved to United States. Amir is a morally ambiguous character because he’s a coward, he’s selfish, he betrays his friend and lies, but he also finds courage to face what he had done wrong and finds salvation. The first part of the book The Kite Runner proves how Amir is not a purely good character. Amir often hangs out with the Hazara boy, Hassan and would tell Hassan that they are friends, but he still feels he’s above Hassan because Amir is Pashtun and Hazara people are considered below the Pashtun people. Amir wouldn’t hang out with Hassan when other Pashtun boys are with him, he also mocks the fact that Hassan can’t read, not considering the fact that Hassan doesn’t have the opportunity to get an education. Amir couldn’t stand Hassan’s intelligence: Amir had written a story about how a man’s tears turn into pearls when they fall into this magic cup, and the story ended with man’s wife dead in his arms on a mountain pile of pearls. When Hassan heard the story, he had enjoyed it, but he also raised a few questions that angered Amir, â€Å"Why did the man kill his wife? In fact, why did he ever have to be sad to shed tears? Couldn’t he have just smelled an onion? † (p. 34). Amir was angry because a mere Hazara boy who couldn’t read had taught Amir something he, an educated boy, didn’t figure out. These few examples that show how Amir is mean and arrogant are nothing compared to what he does to Hassan later on. Assef is a Pashtun boy that truly hates Hazaras and believes that Hazaras should all disappear. When Amir catches Assef raping Hassan, instead of stepping in, Amir runs away and pretends nothing had happened. When guilt started eating Amir up and he couldn’t stand facing Hassan because Hassan reminds him of his cowardly action, he pinned a crime of thievery on Hassan in order to have Hassan evicted from his house. The second part of the book shows that Amir isn’t purely evil despite what he has done. For a while, Amir’s life is filled with the guilt of not saving Hassan from the rape and it kept Amir from being completely happy, even though he found the love of his life in America and got married. Then one day, Amir’s father’s friend, Rhahim, called him to give him a chance to redeem himself. â€Å"There’s a way to be good again† (p. 226). Amir did find a way to be good again. Amir went back to Afghan to find Hassan’s son, Sohrab, to take him with Amir because Hassan and his wife had been shot to death on the street by the Taliban. Amir finds Sohrab with Assef and ends up getting into a physical fight with Assef. Amir basically lets Assef beat him up and while being beat up, Amir feels relieved. â€Å"My body was broken – just how badly I wouldn’t find out until later – but I felt healed. Healed at last† (p. 289). Amir felt that he was being healed from the guilt that has been crawling beneath his skin every single day. He had betrayed his one and only friend, Hassan, lied, and destroyed a chance where Hassan might have left to United States with him and would still be alive. Amir felt that he finally got what he deserved and he felt much better, he had found salvation. And he had afterwards taken in Sohrab as his own son. Amir had been a coward; he had made selfish decisions and ruined Hassan’s live, but if he had been purely evil then he would not have felt guilt, nor would he have risked his life to bring back a mere Hazara’s son. But he had been filled with guilt and he had gone to find Sohrab and redeemed himself. And thus, Amir is a perfect example of a morally ambiguous character.

These benefits include employer contributions to Social Security, unemployment insurance, and workers’ compensation insurance. Altogether such benefits represent about twenty-one and half percent of payroll costs. Social Security Social Security is the federally administered insurance system. Under current federal laws, both employer and employee must pay into the system, and a certain percentage of the employee’s salary is paid up to a maximum limit. Social Security is mandatory for employees and employers. The most noteworthy exceptions are state and local government employees. The Social Security Act was passed in 1935. It provides an insurance plan designed to indemnify covered individuals against loss of earnings resulting from various causes. This loss of earnings may result from retirement, unemployment, disability, or the case of dependents, the death of the person supporting them. Social Security does not pay off except in the case where a loss of income through loss of employment actually is incurred. In order to be eligible for old age and survivors insurance (OASI) as well as disability and unemployment insurance under the Social Security Act, an individual must have been engaged in employment covered by the Act. Most employment in private enterprise, most types of self-employment, active military service after 1956 and employment in certain nonprofit organizations and governmental agencies are subject to coverage under the Act. Railroad workers and United States civil service employees who are covered by their own systems and some occupation al groups, under certain conditions, are exempted form the Act. The Social Security Program is supported by means of a tax levied against an employee’s earnings which must be matched buy the employer. Self-employed persons are required to pay a tax on their earnings at a rate, which is higher than that paid by employees but less than the combined rates paid by employees and their employers. We should clearly be using all available devices for improving the operation of the labor market. Because employees will be compensated for their occupational injuries, it is necessary to take full advantage of the financing of that compensation system in order to create incentives for prevention. The financing arrangements now in use are quite strong, but reinforcing prevention incentives has nev er been viewed as their primary purpose. Recognition of this preventive incentive role and attention to its improvement will serve to improve the occupational health and safety of American workers.

Simplicity and change as a child

I remember finding that first rock. We were digging in the woods behind my house. The entire neighborhood: Alec, Julien, Westley, Blake, Nathaniel, and I. It was a giant pink quartz, the size of a grapefruit. The creases were packed with dirt. Chipping away at the frostbitten ground in November, we began finding more and more rocks: a small white crystal, a green gem, a slab of mica, coral, and fossils. We now had a fairly large pile of artifacts in the shoebox under my bed. I would go and look through them, thinking I had found the biggest treasure in the world. I remember Nathaniel saying how we’d all be millionaires and the two brothers fighting over what we’d spend in on. Did we want a pool for the neighborhood? A new PlayStation? The possibilities were endless, but we’d be famous anyway, the kids that discovered it all! When we had excavated the last of the rocks, we looked them over to clean out the dirt that had plastered itself into all the cracks and holes. We decided to keep it a secret, a dusty cardboard box hidden under all the other old school work under my mattress. I remember sawing back and forth against the plywood. Our swing was going to be amazing. My arm was sore and salty sweat speckled my lip. Summer sun is the most unforgiving heat, dry and merciless. I remember the metallic jingling in our pockets of the coins we had scrounged from the junk drawer. We bought some red paint and metal hooks from the hardware store downtown. I remember the kind chuckle the cashier gave us, a bunch of kids counting out all our quarters and dimes. I remember the countless tries it took to throw the ropes over the outstretched arm of that pine tree. I remember the refreshing rush of air, and not feeling solid ground on my calloused and dirty bare feet. I remember years later, seeing the swing lifeless and dangling, a paint chipping, rope rotting, rust infected memory. I remember the numbness, watery eyes, and stuffy nose. I remember the way snowflakes looked through the orange light of the street lamp. Let the battle begin. We rushed behind the wall of our castle. Westley, Nathaniel, and I, verse all. Packing the snow together with wet mittens I took an iceball to the lip. I remember the strength it took not to cry. The war began to die down; I remember leaning back and falling into the forgiving snow and watching my breaths turn into soft clouds against the piercing black sky. I remember snowflakes on my eyelashes; we were all going to be friends forever. I remember the day Julien moved away. I remember the day Westely started middle school. I remember the day Alec didn’t want to come play football. I remember the day Nathaniel had too much homework to go outside. I remember the day I realized the unforgiving nature of time, the inevitability of change, and the pricelessness of simplicity. Everyone gets the, â€Å"have fun now, ‘cause it won’t last forever kid!† from a sassy grandparent who you don’t want to believe. Sure, nothing can last forever, but I’ve got plenty of time to be a kid. The memories of simplicity that I hold onto so closely are the things that I appreciate most. They are what construct the skeleton that the layers and complications of my life cling to. Every day I feel how precious it is to have modesty, clarity, and sincerity. I remember simplicity.

Since the Project Manager position is a newly created job within LRH Products, it is necessary to assess and describe the various functions assigned to this role, as well as understanding how the concept of project management works within the company (Mullaly, 2008). The specific duties and functions for the project manager at LRH have been broken down here into three component areas: competencies, leadership, and organizational responsibilities. Competencies  The project manager must have the ability to create and execute project plans and revise them as necessary according to changing requirements as well as identifying the necessary resources and assigning staff to specific tasks. This requires day-to-day assessment of the project’s progress along with quality assurance of completed tasks. Another competency is the ability to create a realistic budget and ensure that the team is staying within these financial constraints. In order to do so, a basic understanding of profit & loss statements as well as forecasting, is required. The ability of the project to stay within budget needs to be communicated to appropriate senior managers on a regular basis, as well as timely updates on progress. Meetings with team members must be scheduled and properly facilitated. Information must be disseminated in an appropriate form/forum, with particular attention paid to problems or possible risks which may occur over the course of the project. More detailed presentations may be required, depending on the directives of senior management. Tangible results expected from each project must be appropriately conveyed in order for a project to be considered successful. Because so much of project management is automated through the use of technology, part of the project manager’s burden is to stay up-to-date on the latest software and hardware which aids in the ability to achieve the goals of the position. As well, the project manager must work closely with the IT department of the organization, which can implement necessary additions and changes to the current system. A basic understanding of LRH Products’ network, system, database, client/server architecture, and application programming is necessary for effective communication with the IT staff members. Leadership The ability to lead a team is of utmost importance when performing the duties of project manager. The team members’ development rests squarely on the shoulders of the manager as well as the duty of being able to implement changes within the group as necessary. This begins with being a positive role model and offering mentoring services to the staff. One of the more difficult aspects of leadership is identifying team members who demonstrate opportunities for improvement and handling suggestions to achieve improvement in a tactful and constructive manner. Members of the team must be motivated and encouraged to perform at their utmost capability. This is achieved through the use of acknowledgement of the contributions from each employee as well as a display of appreciation for their efforts. Conflicts will arise, and it is up to the project manager to handle these swiftly and efficiently. The manager must stay in constant communication with the team. Activities planned outside of the normal day-to-day duties of work should also be part of effectively leading and motivating a team. Organizational Responsibilities Part of managing the team’s human resources involves giving performance evaluations to team members as well as working with each to identify and execute personal goals within the organization. Training, beyond the usual provided by the company, may need to be scheduled to appropriately facilitate development. Team members should be encouraged to undertake their own developmental measures, as well. LRH Products will always experience room for improvement of its current processes and a project manager is expected to make recommendations to achieve this. Of course, the standard policies and procedures must be upheld while ensuring compliance of such by the manager’s team members. Another important aspect of the project manager’s duties is to be innovative in developing and designing new projects which will contribute to the company’s overall success. Each project implemented must be completed in a timely manner and within budget constraints to assure success; the six phases from IT project management guidelines include Planning, Analysis, Design, Construction, Test, and Rollout. The project manager must oversee each phase as well as foreseeing and reacting to any risks and changes which may occur along the way. Staying up-to-date on, and utilizing, current technology is a an important component of this aspect. These three areas of project management: competencies, leadership, and organizational responsibilities, all interact. Successful implementation of each area is vital to achieve success in this position.

Author Lane Ashfeldt on the Changing Role of Editors The changing role of editors - An interview with Lane Ashfeldt â€Å"We tend to associate independent publishing with early-career writers getting their first break, since this is the kind of story that has tended to grab the headlines. But another significant group of authors choosing to self-publish are mid-career writers whose contracts for certain titles have expired.†We say on our homepage that our Reedsy professionals â€Å"know the publishing landscape better than a writer knows the taste of hot coffee†. So we like to regularly have their take on it. Today, we interview Lane Ashfeldt, a UK-based freelance editor who has worked both for a small press and directly with authors. She shares her insights on the ever-changing role of editors - whether in-house or freelance - and on the opportunity self-publishing represents for writers (even â€Å"mainstream† ones).Hi Lane, great to have you here. You’re one of our fiction editors on Reedsy and have been freelance for around 4 years. How did you start your editing career?Thanks for asking me Ricardo. I got into fiction by accident, planning to move on, but it stuck. For six years I edited short stories and novels for an indie press, in a variety of styles from crime to literary. I later started writing myself, and taught writing at a UK university.What prompted you to go freelance as an editor?When my book of my stories, SaltWater, was in edits I blogged about the process and some ex-students who had carried on writing after their courses got in contact asking if I’d look at their novels. I couldn’t say no.How do you find having the writer as your employer?I’ve never really thought of it that way. Reedsy refers to editing projects as collaborations and I think this hits the right note, though of course in this context the author is ultimately in control of the process. Which is as it should be - it is their book. I suggest edits, and explain why, but it’s up to the writer whether they choose to act on them. It†™s a very cooperative thing. It’s also private. I don’t identify titles that I’ve edited freelance. (The titles on my Reedsy profile are from a regular job before I went freelance.)Do you see any parallels between your freelance editorial projects now, and when you were a full time editor?I enjoy working direct with authors, and this aspect is very similar. As a commissioning editor I’d meet authors and see their books through from manuscript to hard copy. Now it is more usual to work remotely via electronic means, and generally I only work on the text unless asked for input on submissions or electronic publication routes. One key similarity is, it’s very result-driven – it is all about finishing a piece of fiction, be it for print or ebook publication. Another aspect in common with my experience as a professional novel and short story editor is that over time you build relationships with certain authors and a knowledge of their strengths. It is rewarding when an author asks you to work on multiple projects as the dialogue becomes more fruitful.You have taught creative writing. How does that compare with editing?There is some commonality. But because advanced level university students need to write dissertations and conduct research, there’s a necessary interest, pedagogically, in the process of creative writing; this can distract from the task at hand, ie finishing the book. With project editing, the emphasis is not on â€Å"how to,† if you like, but on getting it done. To as high a standard as possible, obviously.With the explosion in self-publishing, do you feel the role of editors is changing?In the present era of publishing everything is negotiable. Some editors are strong on marketing, some act as curators whose job is to uncover the Next Big Thing in their sphere of interest, some are very much a cog working within the corporate constraints of a major brand. As a full-time editor, I worked with a n indie press. Generally in smaller presses roles are more flexible. Editors may do both copy editing and commissioning  - perhaps also have a say in the design, typesetting, marketing, and PR. Commissioning editors with bigger houses are rarely this hands-on. Often they operate more on the level of curators whose intake is limited to people they know and a group of agents their company has history with, taking on finished projects that someone else (an agent, a professor in creative writing, a literary consultancy, the author) has edited and polished. You could say they operate less as editors (in the conventional sense of the word), more as a gateway to the production and marketing machine behind their brand.But with the increasing variety of publication routes open to writers, there is space for all sorts of editorial approaches to co-exist. Recent projects I’ve worked on as a freelance editor illustrate this - some went on to be self-published, others are taking the ag ent / traditional publication route.Is it still possible to land an agent or publisher with an unedited manuscript?I am sure there are examples out there. The best writers are excellent editors of their own work, and this was true even in the days when some of the editing processes took place heh-hemming over dictation to a secretary. But if hiring a freelance editor adds perspective and strengthens your writing, it is a step worth taking.How do you see the publishing industry evolving in the next few years? Now that independent publishing has been established as a valid option, are more traditionally published authors going to try it?Lots of traditionally published writers have already experimented with publishing their work independently. It skewed towards US-based writers to start with, perhaps because the terms offered to US writers were more favorable, but that is beginning to even up. Both European and US authors working directly with Amazon can now access a royalty of 70% of the cover price on their titles (as against c. 7% working through a publisher) which increases the chance that authors may choose this route.We tend to associate independent publishing with early-career writers getting their first break since this is the kind of story that has managed to grab the headlines. But another significant group of authors choosing to self-publish are mid-career writers whose contracts for individual titles have expired. They are sitting on out-of-print (and professionally edited) novels that readers want to read - maybe even want to reread on ebook, having lost the hard copy - and it is not a lot of work to put up the files on self-publishing websites. A couple of examples: Philip Casey and Jeff Noon. Like Casey, Noon recently had new covers made for all his books and re-released them, along with his new ebook Channel Skin. What is the best thing about editing?For me, it is a similar buzz to writing. I enjoy editing. I love the intensity of it, the fact that you can lose yourself in the work, think about nothing else. Valeria Zurlini essays BAM Cinematek recently chose to feature the films of Italian Director Valerio Zurlini. Zurlini isnt mentioned in many of the putative histories of Italian cinema, yet he is one of the most noteworthy Italian directors of all time. Zurlinis canon of films were most recently screened at The National Gallery of Art, in February of 2001, and have also been screened throughout Canada, and at The Pacific Cinematheque, Lincoln Plaza Cinemas, and Walter Reade Theater within the last two years. Valerio Zurlini was born in Bologna in 1926 to an affluent family. Prior to directing films, Zurlini studied both law and art history. Zurlini said that studying art helped him to develop an intuitive approach to the composition of images, and that going to the movies on a regular basis instilled in him an understanding of cinematic rhythm, presumably reinforced and deepened by his long-standing collaboration with composer Mario Nascimbene (Moller). Zurlini believed that art alone could provide insight into human nature, inspiration, truth and the possibility of redemption, since art endures long after its creator is gone. In relation to Italian film history, Zurlinis films fall in between Rossellini, de Sica, and Zavattinis Neorealism and Bertolucci and Belloccios Young Cinema. Neorealism came in the wake of the Liberation and post WWII democracy. Young Cinema, connected to the student revolt, would occur some twenty years after Neorealsim. Next came the Berlo’s Model of communication in 1960. He argued that the source and receiver should communicate at the same level. With respect to children, Bandura (1977) devised a theory of imitation. According to him, children learn to communicate by observing and i mitating the parents and teachers. They try to make role models out of them. Seeing them behave them in particular manner, they act in the same way. When these associations happen over a period of time, these behaviors get reinforced and get deep seated. Interpersonal communication is a selective, systemic, unique, processual (is an ongoing process) transactions that allow people to reflect and build knowledge of one another and create shared meaning (wood, 2010). Dainton (2011) also argued that interpersonal communication as a process occurring between two individuals, when they are close in proximity, able to provide immediate feedback and utilize multiple senses. A country has the responsibility to decide what outputs to produce and in what quantity, how to produce them and for whom should they be produced. In a market society, the national output is influenced by consumer tastes and the resources and technology available in the country. On the other hand, governments may intervene to compensate for market failures that usually occurs inside a country, or to fund social programs. Here there are some examples that show mixed economies between consumer sovereignty and government interventions. Generally speaking transportation means are controlled by the private sector through companies that provide transportation services such as airlines, company taxis, trains, etc. Inside a country, there also exists public transportation for those people who can not afford the private one. In Peru, specially for the ground transportation, the government plays a fundamental role regulating tariffs, taxes and making sure we have a safe an organized transportation system in the country for the society. Education In a country generally exists public and private education. Usually, private schools tend to have a higher price but also delivering high quality and it is market driven. There are different private schools with different prices in the market. On the other hand, public education is provided by the government to lower socio economic levels in order to make education accessible to the whole population. Government applies procedures in order to maximize its quality. In Peru for example the government is trying to break the poverty cycle incentivizing the rural population to attend school through monetary allowances. There are also regulations in terms of the requirements asked for being a public teacher, the education syllabus of the country, etc. Police Police is a government regulated service to the population which is part of the arm forces of a country. Its function is to assure security to the population. However, in some countries where there is scarce resources allocated to this public entity, private police petrol are created. In Peru even in rural areas where police is not present, communities organize themselves to fight against crime. Energy efficiency of appliances Energy is a strategic scarce resource for a country. Therefore, the importance of having government regulations to control it, its tariffs while having private companies supplying the service in order to assure this service on the long term to the community. Health care coverage Health care is a private and public good also. Government provides health care services to the majority of the population by building public hospitals and providing insurance coverage programs. Usually private health care provides a higher quality service and is accessible to higher socio-economic levels of the population. Television advertising It is a consumer sovereignty good, driven basically by program ratings and market price. Usually, the government owns public television channels but even in those cases the advertising is not controlled. What government usually does is to control advertising of some products such as cigars, or the time and programs where you can advertise such products. We think that in the examples mentioned, a mixed economy of public and private goods is the best to make services accessible to the whole population while having a market driven economy. However, if we customized this question to the Peruvian reality, we think that an example where government could intervene more is the banking regulations entity which should improve the credit policies in order to create a sustainable finance growth. 2. Question 3. This chapter discusses many â€Å"market failures† areas in which the invisible hand guides the economy poorly, and describes the role of government. It is possible that there are, as well, â€Å"government failures† government attempts to curb market failures that are worse than the original market failures? The chemistry of drug metabolism Introduction To describe and explain the chemistry of drug metabolism a basic foundation of knowledge is needed to understand the concepts. Metabolism is one of the methods for analysing the effect of drugs or xenobiotics on the body. It is basically a process of converting lipophilic drugs into more hydrophilic drugs to decrease pharmacological effect and increase subsequent hepatic or renal elimination. So it is essentially a process of inactivation and detoxification of a drug and subsequent elimination of the metabolite formed. The basic knowledge involves the all time classical reactions such as oxidation and reduction and those more advanced reactions including glucuronidation and sulfation. Despite energy being needed to drive such reactions to be in favour, metabolism cannot occur without the complex nature of enzymes catalysing the process. However, metabolism of drugs in human is not solely dependent on the enzymes alone it can be affected by natural micro flora in the small intestines. In an in vitro experiment conducted on ranitidine, it was found that N-oxide was cleaved and is therefore a source of drug metabolism. An alteration in the population of micro flora can affect the of drugs efficacy this is a source of interaction between antibiotics and Microgynon ®. Furthermore, some drugs are bioactivated by metabolism to form active metabolites with a desirable pharmacological function i.e. prodrugs. Unfortunately metabolism can transform an inactive drug or xenobiotic into a biologically active compound which can be carcinogenic to humans. Phenol is a readily formed metabolite of benzene metabolism before catechol and hydroquinone 3,6 which poses a major health concern for humans because it can cause acute myelogenous leukaemia 6 As the great founding father of medicine Paracelsus once said â€Å"all drugs are poison†. Therefore humans and animals have adapted many mechanisms for detoxifying xenobiotics, and these processes are divided into two phases phase I and phase II. It is important to bear in mind that some phase II reactions can occur without phase I metabolism, but phase I and phase II reactions are complimentary and not mutually exclusive. This report describes the chemical reactions of drug metabolism and explains how they occur in vivo. 3.0 Phase 1 Phase 1 metabolism involves the direct enzyme activity on drugs P450 isoform enzymes and esterases are responsible for reduction and hydrolysis of drugs respectively. Each P450 isoenzymes genetic expression varies and can either be inhibited or induced. Knowledge of these drivers of metabolism is essential not only to optimise the use of drugs, reduce harm, maximise benefits in poly pharmacy but also to serve as a template for novel drug development10. P450 and esterase enzymes are mainly found in the liver. Phase I metabolism consists of 3 main reactions: oxidation, reduction and hydrolysis. 3.1 Oxidation 3.1.1 P450 mono oxygenase system 3.1.2 Other oxidation reactions 3.2 Reduction reactions Reduction reactions are mainly interconversion reactions that occur in azo, nitro and epoxide groups and conversion of carbonyl to its corresponding alcohol. Reduction reactions are carried out in the body by P450 isoenzymes, NADH/NADPH reduction systems, carbonyl reductase or aldo-ketone reductase. Azo compounds are generally used in pharmaceutical and cosmetic products. Reduction of an azo group is a classical example of a reduction metabolic reaction. This reaction occurs in the presence of other enzymes and is inhibited in the presence of molecular oxygen. Mechanism of Azo reduction Azo reduction can also occur in the presence of NADH/NADPH system alone within the pH range 3.5-6.08. Mechanism of epoxide reduction This reaction is catalysed by microsomal epoxide hydrolase, a catalytic triad that consists of His 431, Asp226 and Glu 404. Their activity is limited because of a narrow hydrophobic tunnel in the active site and water. A water molecule ionises to form a OH and H+ OH attacks the oxirane ring and thus opens it resulting in formation of vicinal dihydrodiol. This reaction is slow in vitro without acid but in this case epoxide hydrolase catalyses the reaction. Vicinal diols formed are more water soluble thereby terminating genotoxic potential. Hydrolysis Most hydrolysis reactions occur at the ester and amide functional groups, with ester more prone to hydrolysis than amide. Amides are more stable than esters because nitrogen is similar to carbon in size, but less electronegative than oxygen so electrons are pulled into the carbonyl Ï€ electron systems which stabilise its structure. The ease of hydrolysis of esters is used in the development of prodrugs to avoid first pass metabolism, a major problem in orally administered drugs. In vivo hydrolytic metabolism of drugs occurs in the presence of enzymes present in various parts of the body. Hydrolysis of drugs and xenobiotics is generally carried out by esterases mainly in the plasma and intestine and not by P450 systems. The blood, GI tract and liver have the highest hydrolysing capacity. The most significant hydrolysing enzymes are carboxylesterases, cholinesterases, arylesterases and serine endopeptidases. Carboxylesterase is one of the major esterases involved in drug metabolism and xenobiotic biotransformation of drugs with esters, amide and thioester functional groups. In figure 3.0 hydrolysis of ester bond results in benzoylecgonine, a carboxylic acid metabolite. But this is not the only ester group present in the structure. The group present next to the benzene can also undergo metabolism to form benzoic acid. Cocaine in the presence of heroine can generate the toxic metabolite cocaethylene in the presence of alcohol, from concomitant cocaine abuse. Carboxylesterase exists in two different forms hCE1 and hCE2. hCE1 is a more effective metabolic enzyme which transports protein to the endoplasmic reticulum and processes fatty acids and cholesterol in the liver alongside other cholesterol enzymes. The general mechanism of drug hydrolysis in esters and amides is by nucleophilic acyl substitution reactions as shown in figure 3.2.6. Minor structural differences exist between heroine and its metabolites, but their activity differs. Heroin (diamorphine) is converted by hydrolysis to 6-acetylmorphine and morphine. hCE1 mainly cleaves the 3-acetyl linkage to form 6-acetylmorphine. The 6-acetyl linkage is cleaved which later forms morphine with a phenolic -OH and secondary allylic -OH. Diloxanide furorate is a drug of choice and an antiparasitic agent for treating asymptomatic patients with E. histolytica cysts in the faeces and cryptosporidiosis, an acute intestinal amoebiaosis in HIV patients. The drug is orally administered and extensively metabolised by gastro intestinal esterase to form diloxanide and furoic acid, thereby diminishing its effectiveness. This problem is modified by using cyclodextrin that prevents excessive hydrolysis of the drug. Carboxylesterases ability to form a stable complex enhances its presence in the blood and makes it ideal for treating cocaine overdose. It is also considered that as an active site for drugs, this would make it ideal for drug discovery e.g. sarin and VX gas. 4. Phase II Conjugation pathway The phase II conjugation pathway is often a detoxification mechanism. It terminates drug pharmacological activity by changing or masking functional groups in the parent drug or phase I metabolite into a more ionic polar product which aids excretion. The processes that commonly occur in phase II metabolism can be fundamentally divided into 3 groups which are glucuronidation, sulfation and acetylation. The nature and functional group of a drug molecule will determine which one of these processes be in favour e.g. acetaminophen undergoes both glucuronidation and sulfation, however at high doses glucuronidation predominates and at low doses sulfation predominate (Airpine Choonara, 2009). 4.1. Conjugation with sugars Conjugation with various sugars is possible in nature, and novel pathways for xenobiotic metabolism are discovered frequently (Ikenakaa, Ishizakab, Miyabaraa, 2007). However the most important reaction in humans is glucuronidation. 4.1.1 Glucuronidation Glucuronidation is essentially conjugation of a substrate with ÃŽ ±-D-glucuronic acid, shown in figure 4.1.1.1. As the name suggests, glucuronic acid is a derivative of glucose with the 6th carbon being oxidised to a carboxylic acid group. This in combination with the many hydroxyl groups gives glucuronic acid a solubility of 1g/10mL in cold water, which the British Pharmacopeia would class as â€Å"freely soluble† (British Pharmacopeia Commission, 2009) Glucuronic acid is present in vivo as the co-factor uridine 5-diphosphate-glucuronic acid (UDP-glucuronic acid). The reaction of UDP-glucuronic acid with a xenobiotic substrate is catalysed by the enzyme UDP-glucuronosyltransferase (UGT) (Kaeferstein, 2009), and an example of a glucuronidation reaction is shown in figure 4.1.1.2 Figure 4.1.1.2 demonstrates how glucuronidation can occur with a xenobiotic containing an acceptor nucleophilic group (for example COOH, SH or NH2, but in this case OH) (Kaeferstein, 2009) (Sakaguchi, Green, Stock, Reger, King, 2004). The lone pair of electrons on the hydroxyl group attacks at the 1st carbon of the pyranose ring, which is activated because of the adjacent electron-withdrawing oxygens, in an SN2 nucleophilic substitution reaction. The UDP glycosidic bond is cleaved off owing to the good leaving group properties of the phosphate group, and the xenobiotic has reacted with the glucuronic acid to form a ÃŽ ²-D-glucopyranosiduronic acid conjugate. Note that the reaction is known to be SN2 because the formation of an intermediate leads to an inversion of stereochemistry at the anomeric carbon. The resulting glucuronide conjugate has improved solubility due to the hydroxyl and carboxylate groups, and is usually excreted in the urine, although there is evidence to suggest that conjugates with a high molecular weight are eliminated in the bile. However the glucuronides undergo some important reactions within the body which affects their metabolism. A spontaneous intramolecular reaction can lead to esterification of the glucuronide, as shown in figure 4.1.1.4. The newly formed ester carbonyl is capable of reacting with the N-terminal of a protein residue to form a stable imine, i.e. this can lead to irreversible protein binding. Alternatively, depending on which species the glucuronic acid is bound to, nucleophilic substitution can again occur and the xenobiotic will react with the N-terminal of the protein and regenerate free glucuronic acid (Zamek-Gliszczynski, Hoffmaster, Nezasa, Brouwer, 2006). Pharmaceutical companies may therefore try to avoid designing drugs which are predicted to be metabolised by the glucuronidation pathway, not just to increase the half-life of the drug by avoiding conjugation and excretion but also to avoid the potential side-effects that can occur as a result of protein binding, such as cirrhosis of the liver. Interestingly, glucuronidation can also lead not just to metabolites that lose their therapeutic use and are toxic, but some glucuronides can continue to be pharmacologically active and may even be more potent than their parent drug. Morphine-6-glucuronide (M6G) is one such example. M6G and morphine are both potent analgesics M6G, despite having been conjugated with a large polar molecule, still binds strongly to ÃŽ ¼ opioid receptors to provide pain relief to the same extent as morphine. Morphine-3-glucuronide, another metabolite, binds preferentially to NMDA receptors instead, and causes allodynia, myoclonus and seizures (the side-effects associated with opiate usage). Morphine and codeine are so far the only known examples of glucuronides with high activity (Kaeferstein, 2009). 4.2. Glutathione conjugation Glutathione serves as a substrate for electrophilic drugs because of the nucleophilic thiol moiety on the cysteine residue (thus glutathione can be referred to in reaction pathways as simply GSH). GSH conjugation therefore involves a nucleophilic attack of the sulphur atom onto drugs with electrophilic carbon atoms, i.e. those bound to good leaving groups such as halogens, sulphate and nitro, as well as activated carbon atoms in ring strained systems such as epoxides and ß-lactones (Zamek-Gliszczynski, Hoffmaster, Nezasa, Brouwer, 2006). Conjugation leads to a thioether bond being formed between GSH and the drug molecule. Following this reaction, conjugates are typically metabolised further to yield more polar molecules which are better excreted in the urine and bile (Zamek-Gliszczynski, Hoffmaster, Nezasa, Brouwer, 2006). Figure 4.2.4 shows the possible biotransformation reactions of a glutathione conjugate. Transpeptidase and peptidase convert glutamate to NH2 and remove glycine, respectively. NH2 is then a target for N-acetylation (mentioned in section 4.4). Alternatively, two molecules of glutathione can react together to form a disulfide bridge, in the process donating hydrogen atoms to reduce another molecule. This is usually utilised in vivo when glutathione acts as an antioxidant (Forman, Zhang, Rinna, 2009), but also plays a part in drug metabolism as seen in the denitrification of the antianginal drug, glyceryl trinitrate (GTN) in figure 4.2.5 (Ji, Anderson, Bennett, 2009). To reiterate, GSH reacts with highly electrophilic species in the body. This prevents drugs with electrophilic groups from attacking important nucleophilic centres in biological molecules, such as DNA and proteins, which could lead to toxicity. This is explored further in section 5 where the consequences of insufficient glutathione conjugation of paracetamol metabolites are looked at. 4.3. Sulfation Sulfation is one of the classical processes of phase II metabolism. It allows the biotransformation of numerous xenobiotics and metabolites from phase 1 (shown in figure 4.3.1) to be sulphate conjugates. This gives protection against toxicity or the potential toxic effects from the numerous xenobiotics and metabolites not being conjugated. It also produces more polar, more water soluble metabolites, which means they are more easily and readily excreted in urine or bile. The sulphate conjugate possesses such advantageous properties by having a low pKa, allowing an increased aqueous solubility and excretion. It is an important reaction for drugs and hormones that contain the phenolic functional group to be metabolised by conjugation to a sulphate group examples include steroid hormones, catecholamines, neurotransmitters, thyroxine, bile acids and phenolic drugs. Examples of drugs and xenobiotics with a phenolic group attached: The chemistry behind the sulfation conjugation reaction emphasizes the important key features of the system. This includes the two enzymes sulfatase and sulfotransferase, alongside the co factor 3†²-phosphoadenosine 5†²-phosphosulfate (3†²-phosphoadenylylsulfate, PAPS) which plays an important role in sulfation conjugation. The availability of PAPS and its precursor inorganic sulphate determines the reaction rate as the total amount of sulphate is limited and can be readily used up. PAPS is formed enzymatically by ATP and inorganic sulphate. The enzyme sulfotransferase transfers the active sulphate from PAPS to the xenobiotic or a phase 1 metabolite forming the sulphate conjugate (VL Verdugo D, 2004). Sulphate conjugation is a reaction principally of phenols and to a lesser extent alcohols to form highly ionic polar sulphates. Sulphate conjugation is also important for steroids because steroid sulphates are not capable of binding to their receptor and so this reduces its biological activity. Sulfation of alcohol generates a good leaving group and can be an activation process for alcohols to produce a reactive electrophilic species. Mechanism of sulfation conjugation an electrophilic substitution reaction: The oxygen of the OH has a negative inductive effect on the benzene ring so it withdraws electrons towards it making it a more reactive nucleophile It attacks the electrophilic sulphur of the sulphate group of PAPS The hydrogen of the OH bond leaves in exchange for the sulphate group and UDP acts as a good leaving group This forms the sulphate conjugate which is soluble and readily excreted via the kidneys 4.4. Acetylation Conjugation Acetylation is also an important reaction in phase II metabolism as the majority of drugs contain a primary amine functional group. It is a major route for the biotransformation of hydrazine and aromatic amines. This means that acetylation of the arylamine or phase 1 metabolites can occur more easily to reduce their biological activity (Garcia-Galan Diaz-Cruz, 2008). The limitation of acetylation is that it produces conjugates that are less water soluble (Zamek-Gliszczynski, Hoffmaster, Nezasa, Brouwer, 2006) as well as it does not work for drugs containing secondary amine groups. The aim of acetylation is to convert the primary amine moiety into an amide because amides are more stable as peptide bonds are more resistant to hydrolysis. Like glucuronidation and sulfation this reaction is highly specific because of the nature of the enzyme involved. The main players of acetylation conjugation are N-acetyltransferase and the co factor acetyl Coenzyme which is a thioester . The reactio n undergoes electrophilic substitution similar to Friedal-Craft acylation. The NH2 attached to the aromatic ring makes it much more reactive and electron donating. NAT helps to transfer the acetyl group (CH3CO) obtained from Co enzyme A (CH3COSCoA) to conjugate with the drug at the amine site forming the amide bond. H-SCo-enzyme acts as a good leaving group. Mechanism of acetylation conjugation: The lone pairs of the nitrogen of the primary amine of sulphonamide attack the carbonyl carbon of the acetyl group of the acetyl coenzyme A. In this reaction nitrogen acts as a nucleophile, donating the pair of electrons to the electrophilic carbonyl carbon. The carbonyl carbon (ÃŽ ´+) is activated by the electron withdrawing oxygen (ÃŽ ´-) making it more susceptible to nucleophilic attack. This forms a temporary tetrahedral intermediate, which falls back to form an amide bond and SH-CoA acts as a leaving group. As a result the acetyl conjugation of sulphonamide is formed, and this is readily excreted via the kidneys. 4.5 Stereo selectivity Stereo selectivity is classed as a fundamental aspect of drug metabolism ever since the tragic case of the drug thalidomide. This has provided a broader knowledge on the understanding of drugs and xenobiotics and also the importance of their stereochemistry properties. As mentioned in section 4.1.1 (glucuronidation), drug metabolism may lead to stereochemistry inversion of substrates during the various reactions that occur. An example of how the understanding of stereochemistry in xenobiotic metabolism has practical applications can be seen with the non-steroidal anti-inflammatory drug ibuprofen. It has been found that in vitro, only the S-isomer is pharmacologically active in inhibiting cyclooxygenase enzymes. However in vivo the metabolism of ibuprofen is complex, involving glucuronidation at the acyl group and hydroxylation at the 2 and 3 positions, but most importantly the metabolism of the 2 enantiomers differs because there is a unidirectional enzymatic conversion of the R-isomer to the active S-isomer. (Chang, et al., 2008). The metabolism of ibuprofen is summarised in figure 4.5.2. For this reason drug manufacturers typically produce a racemic mixture of ibuprofen for administration to patients, since the R-isomer will be converted within the body, and producing an enantiomerically pure sample would be needlessly expensive. 5. Micellaneous Amino acid conjugation is important for metabolising, solubilising and eliminating carboxylic acids through the urine because it produces very soluble conjugates. Amino acid conjugation mechanism e.g. benzoic acid (Xu, et al., 2007): The carboxylic group of the benzoic acid is first activated by ATP to the AMP ester This is then converted to the corresponding coenzyme A thioester with CoASH. These first two steps are catalysed by acyl Coenzyme A synthase enzyme The appropriate amino acid N-acyltransferase then catalyses the condensation of amino acid and Coenzyme A thioester to form the amino acid conjugate. Methylation conjugation: Even though it is not a common reaction for most drugs and xenobiotics, it is worth mentioning methylation because it is the most common biochemical reaction for endogenous compounds such as catecholamines (Strous, et al., 2009). Methylation plays a key role in the inactivation of amines such as norepinephrine, serotonin, dopamine and histamine, and is also involved in the biosynthesis of epinephrine and melatonin. A source of methyl comes from the high energy nucleotide S- adenosylmethionine (SAM) which is transported by cathecol-O- methyltransferase. However, it has been reported that methylated conjugates do not have improved water solubility (a similar disadvantage to acetylation). Methylation mechanism the nucleophilic substitution of norepinephrine: The lone pair on the electronegative oxygen of norepinephrine (R-OH) attacks the CH3 of SAM The bond between the sulphur and carbon breaks (S-C) Drug Toxicity The toxicity associated with acute paracetamol overdose is due to its metabolism processes. In the human body, paracetamol is mostly metabolised 30% by the sulfation pathway, 60% via glucuronidation and the remaining 10% being either excreted unchanged in the urine or undergoing CYP450-dependent oxidation as shown in figure 5.3 to form N-acetyl-p-benzoquinoneimine (NAPQI) (Airpine Choonara, 2009). NAPQI contains an electronically activated ring system, capable of attacking nucleophilic molecules such as N atoms in cellular macromolecules and causing cell damage. However NAPQI will preferably attack the more nucleophilic sulphur atom of glutathione and therefore will also undergo phase II metabolism to form inactive conjugates a schematic summary of the metabolism of paracetamo In overdose situations, the glutathione supply is used up as it is conjugated with the excessive NAPQI in the system. This leaves the rest of the NAPQI free to bind irreversibly to proteins in hepatic liver cells (since P450 metabolism occurs predominantly in the liver) and this cause liver necrosis. Without the detoxification capacity of the liver, the human body will typically die within 2 weeks (Airpine Choonara, 2009). With the chemistry of paracetamol metabolism in mind, it is easier to understand why some patients are classed as â€Å"high-risk† and thus more susceptible to paracetamol overdose: Recent alcohol (ethanol) consumption causes induction of the P450 enzyme involved in the formation of the NAPQI molecule; this leads to an increased quantity of NAPQI being produced and therefore the bodys supply of glutathione for conjugation is more rapidly used up leading to toxicity. Other drugs which induce the same P450 enzymes will have the same effect. Eating disorders such as anorexia nervosa lead to a poor diet and therefore decreased synthesis of glutathione in vivo, so NAPQI detoxification conjugation can be overwhelmed at lower concentrations of paracetamol consumption.

Personal Narrative- September Eleventh :: Terrorism Terror

Personal Narrative- September Eleventh No one knows what tomorrow will bring or may not bring. In the course of the past year, this idea was extremely prevalent. Because of the September tragedy, many people were uneasy and unnerved about daily situations. This concern caused people to look at things in a new, almost like an â€Å"enlightenment† initiator. September the eleventh is a day I will always remember; not only for the obvious reason, but for the important way it affected my feelings towards my family. How could anyone forget the day America was attacked? I remember driving to school on the day of the attack. The Texan breeze brushed fallen leaves across the roadway as I pulled into my high school parking lot. It was a day like any other, or so I thought. During my first period class, I was supposed to play my solo and ensemble piece for one of the band directors so they could help me. As I sat in the office and played a concerto by Mozart on the French horn, the band director listening to me got a cell phone call. I stopped playing so she could answer the phone call. It was the other band director’s wife reporting the attack. The first feeling that came to me was unbelief, but as I walked around the school and saw teachers with their TV’s stationed to news channels I saw the horrific pictures. I later found out that some of my relatives were in the World Trade Center Towers that day. No one knew if they were all right. My family eventually got a phone call reporting that they had gotten out o.k. Another family member was affected by the attack. My dad, who is in the Air force, received a notice and was re-stationed to Minot, North Dakota. He was there for about a year and a half. He may be shipping out soon to go to the Middle East. With everything that had happened, there is no way that the attacks could not have affected me.

Of this total, liquids account for the largest share of the 230m barrels (37%) followed by coal (23%) and natural gas (21%), leaving a 19% contribution from nuclear and sustainable sources. Of the liquids, however, transport use accounts for more than half (51%), with the remainder going to industry (32%), residential and commercial (11%) and power generation (6%). The residential and commercial share is mainly accounted for by oil-fired central heating, and the small power generation share by gas turbines running to meet peak loads. It follows that the transport sector share of the world energy market is just over half of 37%, in other words some 19% or just under one-fifth of the total. It should also be borne in mind that the transport sector is itself divided into light-duty vehicles (privately-owned passenger vehicles and light commercial vehicles), heavy-duty vehicles, and other transport applications (aircraft, ships and diesel railway engines). The light-duty and heavy-duty sub-sectors take about 40% each, and the other applications some 20%. The search for improved efficiency in the transport sector must therefore embrace both light-duty and heavy-duty road vehicles.
